Director of HomeCare
Summary: The Director of Home Care is a registered nurse that supervises the home health and hospice department. The Director provides high level leadership to nursing and clerical staff.
Normal Work Hours: 8:00 - 4:00 Monday - Friday with the possibility of weekend and after hours work due to trainings, appointments with clients, clinics, board meetings, speaking engagements, or emergency preparedness events. On call rotation (every 4-6 weeks).
Necessities:
- Must keep the identity and health information of all clients confidential at all times.
- Must maintain valid driver's license, registration, and vehicle insurance and be able to drive his/her vehicle for any work assignments.
- Must follow DCFS mandated reporting requirements.
- Must possess strong leadership ability and excellent people skills.
- Must be patient, organized, and financially savvy.
- Must successfully pass a fingerprint and background check.
- Must understand and speak fluent English.
Physical Demands of the Job:
- Must be able to bend and stoop repetitively.
- Must be able to be on one's feet for prolonged periods of time.
- Must have manual dexterity and control of hands in order to provide immunizations, blood draws, IVs, etc.
- Must be able to lift required nursing equipment which is normally 20 pounds or less.
- Must be able to work at a computer station for prolonged periods of time.
- Must be mobile enough to fulfill the job requirements which include the ability to get in and out of the vehicle, traverse rough terrains, and enter spaces which may be totally handicapped inaccessible.
Environmental Factors:
- Must be able to work in confined spaces as needed.
- Must be able to work in conditions of extreme cold or heat as needed.
Essential Job Functions (Duties and Responsibilities):
- Keeps and maintain records of case assignments and case management.
- Establishes methods for coordination of care by all disciplines.
- Establishes staffing patterns which reflect the quality and quantity of various professional personnel necessary to plan, provide, and supervise the care rendered to patients and families.
- Makes on-site supervisory visits as directed or necessary.
- Possesses knowledge of all aspects of Home Health and Hospice including Medicare rules and regulations for Home Health and Hospice.
- Works with clinical nursing staff to solve day-to-day problems regarding patients and/or their families.
- Provides Home Health and Hospice protocol guidance to nursing personnel.
- Assists intake of Home Health and Hospice patients and assists in scheduling nursing and therapy for those patients.
- Assists with QA of Home Health and Hospice charts
- Knowledge of ICD-10 coding and/or be certified in it
- Knowledge of OASIS and HIS
- Knowledge of and completes QAPI for both Home Health and Hospice
- Knowledge of QuickBooks and enters all EFT payments from Medicare into QuickBooks for both of Home Health and Hospice and ability to monitor financial status for Home Health and Hospice
- Monitors Home Health and Hospice budgets
- Monitors and approves Home Health and Hospice timekeeping
- Acts as a resource person for in-service education and training programs for Homecare nursing and clerical staff and community organizations.
- Stays current of innovative practices and current trends in Home Health and Hospice nursing; recommends application, adaptation or modification of such practices to meet the needs of the local health department and populace it serves.
- Plans, assigns, and reviews the work activities of subordinate nursing staff engaged in providing nursing services.
- Assists in writing press releases and meets with the media as needed.
- Attends leadership meetings and regional homecare meetings.
- Directs and coordinates the activities of Home Health and Hospice staff and contractual staff
- Initiates and completes contracts with insurance companies, VA etc.
- Obtains PS&R from CMS
- Knowledge of EMR software and how to find answers to staff questions
- Hires, disciplines, evaluates, and fires subordinate staff as needed with the consent of the administrator.
- Orients all newly hired staff for this department.
- Establishes effective working relationships with the general public, community leaders, members of professional organizations and governmental officials.
- Analyzes and evaluates nursing services to improve the quality of patient care.
- Is cross trained and serves as a back-up for any activities relevant to programs as needed (this includes clerical, billing and seeing patients).
- Attends all regular Board of Health meetings and Professional Advisory Committee meetings and prepares reports for such meetings.
- Serves as a back-up supervisor in the absence of other department supervisors
- Performs other duties as assigned.
Education and Experience Requirements:
- Requires graduation from a school of nursing approved by the Department of Professional Regulation; and an Illinois license as a Registered Nurse by examination; and 2 years of full time home health or hospice nursing experience, or A bachelor's degree in nursing and one (1) year of full-time experience in home health or hospice.
- Possesses and maintains current CPR certification.
